Urology at the Harborne Hospital

I hold an outpatient clinic and operating list at The Harborne Hospital, HCA Healthcare's flagship private hospital in Birmingham. For me, it is the natural home for complex stone surgery as it combines the latest endourology equipment with level-3 intensive care, so I can take on the larger and higher-risk cases that would not be safe to perform in the private sector elsewhere.

A hospital built for complex surgery

The Harborne Hospital opened in 2024 as HCA Healthcare UK's first hospital in the Midlands, built at a cost of around £100 million on the Queen Elizabeth Hospital Birmingham campus in Edgbaston. It is an eight-storey, purpose-built facility with 50 private en-suite rooms, day-case beds, four operating theatres including a hybrid theatre, and a full imaging suite with CT, MRI, fluoroscopy and ultrasound on site.

Critically for stone surgery, the hospital has a level-3 intensive care unit and high-dependency beds. Most stone operations are straightforward and patients go home the same day, but for staghorn stones, large stone burdens and patients with significant other medical problems, having an HDU and ITU in the same building changes what can be offered safely. It is the main reason I refer my most complex cases here.

The Harborne is a joint venture between HCA Healthcare UK and University Hospitals Birmingham NHS Foundation Trust (UHB), sited on the Queen Elizabeth Hospital campus, with two of its eight floors and 72 beds dedicated to NHS patients and run by UHB. As Deputy Chief Clinical Information Officer at UHB, I work across both the NHS and private sides of urology in Birmingham. That continuity matters for patients: the consultants, imaging and pathology operate to the same standards, and complex cases can be discussed through the regional multidisciplinary team for kidney stones and through our private team.

Kidney stone surgery at The Harborne

This is where I carry out my most advanced stone work. The available techniques include:

  • Thulium fibre laser lithotripsy. The hospital's laser platform uses SuperPulsed thulium fibre technology (the Olympus Soltive system), the first major advance in laser lithotripsy in two decades. It fragments and dusts stones into very fine particles faster than the older holmium lasers, with virtually no stone retropulsion, which means more complete stone clearance during flexible ureteroscopy.

  • Suction sheath ureteroscopy. I use suction (aspiration) access sheaths during flexible ureteroscopy, which continuously remove stone dust and small fragments as the laser works. This improves stone-free rates and helps keep pressure inside the kidney low.

  • Mini PCNL and standard PCNL. For larger or harder stones I perform both mini percutaneous nephrolithotomy (a smaller tract, less bleeding, quicker recovery) and standard PCNL for the largest stone burdens. The on-site HDU and ITU make The Harborne my preferred site for these cases.

  • Complex and staghorn stone surgery. Large branched stones often need a planned, sometimes staged, combined approach. The hybrid theatre and high-quality on-table imaging support this.

I perform around 50–60 PCNL and mini-PCNL cases and around 50–70 ureteroscopy procedures each year across my practice, with the most complex concentrated here.

Aquablation for prostate enlargement

For men with an enlarged prostate (benign prostatic hyperplasia), The Harborne offers Aquablation therapy — a robotically controlled, heat-free waterjet technique that treats the obstruction precisely while aiming to preserve sexual function. It is one of the newer options for BPH and a good example of the advanced technology available on site.

The latest imaging on site

Rapid, accurate imaging underpins good stone care. The Harborne has modern CT, MRI and ultrasound in the same building, so a patient with renal colic or blood in the urine can often be scanned and have a treatment plan the same day rather than waiting weeks for separate appointments.
